Medical Record Requests
We’ve seen a sharp increase in record requests due to telehealth access and disability applications. Each request requires provider review and admin processing. We kindly ask for your patience as we work through these:

We pride ourselves on providing fast and efficient response to any client requests. We encourage you to use the patient portal, which is accessed by simply responding to the text reminder you receive for each appointment. Our staff receive portal messages in real time. They can perform a wide array of requests such as scheduling an appointment, answering questions, processing refill requests, changing pharmacies, etc., while making it faster and easier for you to communicate. Staff also respond to emails at centeredcs@gmail.com quickly if you prefer this method. Or call our front desk if preferred (though online options are faster).

New pharmacy name, phone number, medication name, and dosage - Patient Portal: Reply to your text appointment reminder through the patient portal with the same info
